Helping Hands



In conjunction with Glendale Community College, Mesa Community College and John C Lincoln Health Networks, Helping Hands has monthly gatherings for moms. This time together is a gathering point for single moms to learn from special presentations and meet each other for the purpose of initiating mutually supportive relationships. Our goal is to expand our network meetings throughout the community.

**This project is based on solid research. In conjunction with Arizona State University West and Valley of the Sun United Way, Helping Hands for Single Moms conducted a successful research project in 2005. The goal was to determine how single moms in college can form productive and mutually supportive relationships.

SCHOLARSHIP REQUIREMENTS

* Attend college
* Take a minimum of nine credit hours per semester
* Maintain a minimum 2.5 GPA
* Attend and participate in the Single Moms Network meetings
* Be a responsible and responsive program participant
* All applicants are required to live in Maricopa County, Arizona

Children's Volunteer Health Network



Just for Grins: The CVHN Mobile Dental Clinic

In 2008 CVHN will launch its first mobile dental clinic program. This program provides preventive dental care and education to local school-aged children on the free or reduced lunch programs. Dental decay is the single most common chronic childhood disease. Every week, CVHN receives calls to help children with severe dental problems. Children as young as 5 need root canals and multiple teeth extracted. Left untreated, dental decay has short and long term negative effects including poor overall health and untreated decay is linked to diseases such as diabetes, heart and lung disease and strokes. Sadly, dental decay is almost entirely preventable. The Just for Grins mobile dental clinic will provide the dental treatment and education needed to transition kids from costly and painful emergency dental care to cost-effective preventive care, allowing these kids to remain healthy and be productive students. The first Just for Grins program will initially visit Walton County elementary schools and serve children K-3 on the free or reduced lunch programs. Sponsorship opportunities are available through CVHN.


Healthcare Referral Form & Process

The school teachers identify children who need medical attention and send them to the school nurse. The school nurse has a parent or guardian fill out the simple CVHN referral form and she faxes it to CVHN. The CVHN Case Manager matches the child with the appropriate healthcare provider and schedules an appointment. The child is then picked up by a volunteer at the school if the parent is unable to transport the child to the doctor’s office. Any follow-up care is scheduled by the CVHN Case Manager.

Free Prescription Discount Card



What is a Prescription Discount Card?
A card that allows people who do not have other prescription drug coverage to obtain discount savings on prescriptions ranging from 10-50%.

Which prescription drugs are discounted?
90% of FDA approved generic; brand name and mail order drugs are discounted.

Which pharmacies participate?
Most chain stores including CVS, Wal-mart and Walgreen's as well as many smaller "mom and pop" stores, about 95% of pharmacies nationwide participate in the program.

How do I know if my pharmacy participates?

What if I have a pre-existing condition?
You will not be asked any questions about your health, income or age – you may use the card immediately.

Is there a maximum number of times I can use the card?
No, use it as many times you need.

Is there a cost for the discount card?
No, we are giving the discount card to you for free, even though these cards retail for as much as $5.95 per month.

Can I use it when I travel?
Yes, the card can be used nationwide.

Can my family and friends use the card?

Can I use my card with Part D of Medicare?
Yes. You can use your card in the donut hole portion of Medicare Part D.

Can I use the discount card with my current insurance plan?
You may use this card when your prescription insurance runs out or if it doesn't cover a specific drug. You cannot use the discount card with a co-pay prescription insurance plan.
